What do botox results actually look like — and when do they appear? Forehead, frown lines, crow’s feet, brow lift, jaw slimming. This guide covers realistic outcomes by area, the actual timeline, and how to read before and after photos without being misled.
When do botox results show? First changes appear at day 3–5. Full effect at day 10–14. Don’t assess the outcome before day 14 — early photos don’t represent the final result. Masseter jaw slimming takes longer: visible at 6–8 weeks, peak at 3 months.
How to Read Botox
Before and After Photos
Botox before and after photos are genuinely useful — and easy to misread. The same lighting issue that applies to filler photos applies here: harsher light in the “before” makes every line more visible. Softer light in the “after” smooths everything regardless of treatment. Same rule: consistent lighting, same angle, no difference in expression.
The other issue is timing. Day 2 botox photos don’t show the result — they show early onset, which looks uneven because different muscles respond at different rates. The real result is at day 10–14 when everything has settled. Any clinic showing dramatic transformations labeled “3 days after” is either showing the wrong measurement point or adding filters.
Assess at day 14 — not earlier
Full botox effect takes 10–14 days. Day 1–5: partial effect, muscles still responding unevenly. Day 7: most of the effect present. Day 14: full, settled result. This is the correct comparison point. Before photos taken at consultation + after photos taken at day 14 = accurate representation.
Look for natural movement in the “after”
Good botox doesn’t eliminate expression — it reduces the depth of wrinkles during expression. An “after” photo where the person has zero forehead movement at all suggests overcorrection, not ideal results. The goal is lines that are softer when the face moves, not a face that can’t move.
Match the case to your concern and muscle strength
A patient with deep static forehead lines (visible at rest) needs a different protocol than someone with light dynamic lines (only visible when moving). Results vary accordingly. Strong masseter muscles produce more dramatic jaw slimming than mild cases. Find cases similar to your starting point, not just the most impressive photos.

Forehead lines before and after
Before: horizontal lines visible with any upward expression, possibly visible at rest in older patients. After (day 10–14): lines significantly softer with expression, nearly invisible at rest. The forehead still moves — just less dramatically. Result lasts 3–4 months. Patients who return consistently before movement fully returns often develop progressively smoother foreheads over time as the muscle weakens slightly with regular treatment.

Frown lines (11 lines / glabella) before and after
Before: vertical lines between brows at rest or with concentration. After (day 10–14): lines significantly reduced or eliminated. The glabellar muscles are strong — this area responds reliably to botox and often shows the most dramatic improvement. Static lines (present at rest) improve with treatment but may not fully disappear — depth of static lines reflects accumulated damage, not just muscle activity.

Before: fan-shaped lines at outer eye corners with smiling. After (day 10–14): lines significantly softer, eye area looks fresher. One of the most reliable botox results — the orbicularis oculi responds consistently and quickly. The eye still smiles; the lines just don’t fan out as deeply. Duration 3–4 months. Day by Day
Most patients arrive expecting to see results within 24 hours. That’s not how botox works. The toxin needs time to bind to nerve terminals and inhibit acetylcholine release — a biological process that takes days, not hours.
What actually happens day-by-day is more nuanced than most clinics explain. Understanding the timeline prevents the anxiety that comes from looking in the mirror on day 3 and seeing uneven, partial results — and assuming something went wrong.

Day 0
Injection — no visible change yet
Mild redness and occasional small bumps at injection sites resolve within 2–4 hours. Some patients notice the first subtle reduction in movement by end of day — most don’t. Normal activity resumes immediately. Avoid rubbing the treated area for 4 hours, intense exercise for 24 hours.

Day 1–3
Early onset — partial, uneven effect
Some muscles begin relaxing earlier than others. The result looks uneven at this stage — one side may appear to have responded more, or movement is partially reduced rather than smoothed. This is not asymmetry from the injection; it’s the natural progression of botox onset. Don’t judge the result here.

Day 5–7
Main effect settles in
Most of the botox effect is now present. Lines are visibly softer, movement is reduced. Still not the final result — some further settling happens over the next week. Patients who check at day 7 usually feel reassured at this point even if they were anxious earlier.

Full effect — assess here
The final, settled result. This is the correct comparison point for before and after photos. Any residual asymmetry visible here is the actual outcome — not an artifact of the settling process. Month 3–4
Movement returns — plan maintenance
Acetylcholine release gradually resumes as nerve terminals regenerate. Movement starts returning, lines become slightly visible again. The ideal maintenance window: return before full movement is back — this keeps results more consistent and may reduce the units needed over time. Botox works on dynamic wrinkles — lines created by muscle movement. It doesn’t work on everything people sometimes expect it to.
