The photo taken right after filler isn’t the result. That’s swelling. It peaks at 24–48 hours, then resolves over the following week. The patient who walks out looking slightly overfilled is not overcorrected — they’re swollen. The final result appears at week 2.

Lighting is the other issue. Harsher light in the “before” makes hollows and lines more visible. Softer light in the “after” makes everything look smoother regardless of treatment. Credible before and after photos use identical lighting, same angle, no makeup in either shot, and show the result at week 2 minimum — not day 1.

Check the timestamp — week 2 minimum

Day 1 photos show swelling, not results. Day 3 photos show peak swelling. Week 1 photos show early integration. Week 2 is when you can actually evaluate the outcome. Clinics showing “immediate after” photos as the result are either uninformed or deliberately choosing the most dramatic-looking moment.

Consistent lighting and angle

Different lighting between before and after produces the appearance of improvement without any. Same rule as Ultherapy photos — both images should be taken under identical conditions. If the “after” photo looks significantly brighter or softer, that’s not the filler working.

A dramatic cheek volume restoration in a 55-year-old with severe volume loss doesn’t predict your result if you’re 38 with mild hollowing. Ask clinics specifically for cases similar to your starting point. The most impressive photos are rarely the most relevant ones.

Results Timeline —
Day by Day

Most patients know filler works immediately — fewer know that the immediate result isn’t the real one. The timeline below is what patients at Yaan Clinic actually experience, not the optimistic version.

For volume fillers that include collagen-stimulating components (Radiesse or combination protocols), there’s a second phase of improvement at months 2–3 as new collagen develops. The initial HA volume gives the immediate result; the collagen stimulation gives the sustained one.

  • Day 0

    Immediate — volume visible, swelling begins

    Volume improvement is visible right away. Swelling starts within hours. Most patients leave the clinic looking slightly fuller than the final result. Normal activity resumes same day. Avoid heat, intense exercise, and massage for 24 hours.

  • Day 1–3

    Peak swelling — don’t judge the result yet

    Swelling peaks 24–48 hours after treatment. The face looks more filled than the final result. Bruising is most visible in this window, particularly in the under-eye area. This is not overcorrection — it’s swelling. Most patients feel comfortable going out but know it’s not the final look.

  • Week 1

    Swelling resolves — filler integrates

    Swelling is largely gone. Filler is integrating with surrounding tissue — settling into its final position. Bruising in high-vascular zones (under-eyes, lips) is resolving. The shape is getting closer to the real result but isn’t there yet.

  • Week 2

    Final result — assess here

    This is when the before and after comparison is meaningful. Filler is fully integrated, all swelling is gone, and the shape reflects the actual placement. Most patients book a follow-up at week 2 if any touch-up is needed. This is the baseline for deciding whether more product is appropriate.

  • Month 2–3

    Collagen phase — additional improvement

    For protocols including Radiesse or PLLA collagen stimulators, a second wave of improvement develops as new collagen forms in the treated zone. The filler volume is still present, but the skin quality and structural support around it improves. This is the phase that makes collagen-stimulating fillers worth the higher cost for structural zones.

What Fillers Can’t Do —
Setting the Right Expectations

Filler before and after photos show volume restoration and contour definition. They don’t show structural lifting — that’s a different procedure. Knowing the limit saves patients from the disappointment of expecting the wrong outcome.

What filler does

Restores lost volume. Defines contours — jawline, chin, nose. Reduces shadow from hollows (under-eyes, temples). Softens lines by supporting the tissue above them.

What filler doesn’t do

Tighten sagging skin. Lift structural tissue (jowls, brow drop, neck laxity). Replace collagen lost to aging. Produce the same result as surgery for significant structural descent.

When to combine filler + lifting

Many patients need both. Filler for volume loss, Ultherapy Prime for structural sagging. These can be done in sequence in the same visit. The combination produces results that neither approach alone can achieve.

The pillow face problem

Over-filling — too much product in too many areas — creates the “pillow face” look that filler gets blamed for. It’s not the filler. It’s the volume placed without considering facial proportions. The answer is less product in better positions, not avoiding filler.
